BEST Health Plan members do not have a copay for the first six office visits after July 1, 2016 that are primarily to treat one or more of the following chronic conditions: Diabetes, Asthma, Hypertension, Clinical Depression, Congestive Health Failure, and/or Coronary Artery Disease.

Please do not collect a copay and please code this correctly!
